information.
The records should also include information about your dog: Its name, breed, sex, weight,
height, and any acute or chronic medical conditions for which your dog is being treated.
Include information about your dog's identifying markings, its microchip, and registration number
with local officials where you live. Finally, include general information about the veterinary clinic
that usually treats your puppy – the name, address, telephone number, web site, and email
address of your puppy's doctor.
What Your Puppy's Online Health Records Should Include - After entering all the general
information about you, your dog, and your dog's veterinary clinic in your puppy's online health
record information, go on to enter medical information about your pet so it can be accessible to
anyone treating your dog.
What kind of dog food does your dog eat? Is he or she under any dietary restrictions? Does
your puppy have allergies to any foods or substances?
Any temporary or ongoing medical conditions your pet suffers from should also be entered
into your puppy's online health record. Include dates of treatment, a description of the
treatment received, and any medications your puppy is taking.
